Nursery medicine consent form and dose record

A free printable medicine consent form for nurseries, with a record of every dose given. Prescribed and non-prescribed medicines.

The printed medicine consent and dose record sheet

What's on it

Required by the EYFS paragraphs 3.61–3.62 (childminders 3.60–3.61)

  • The parent’s written permission for this medicine
  • If prescribed, who prescribed it
  • A record of each dose given
  • When the parent was told about each dose

Good practice

  • The medicine’s strength, the dose and how it is given
  • When it is given, and the dates
  • The last dose given at home
  • Who gave each dose, and who checked it

What the EYFS asks for

The EYFS asks for written permission from a parent for each medicine before it is given, a written record every time, and for the parent to be told the same day. Prescription medicines only when prescribed for that child by a doctor, dentist, nurse or pharmacist.

Skip the paper: it’s easier in Coat Peg

The same record, kept on the room’s device as you go, with nothing to print, file or chase.

  • The parent gives consent in their own app, so there is no form to chase at the door.
  • Each dose is checked against the consent, the dates, the minimum gap and the daily maximum as it is recorded.
  • The parent is told about each dose the moment it is recorded.
